MindLedger is designed exclusively to help licensed mental health providers collect structured pre-appointment information from patients in a consistent, digital format. The platform facilitates documentation — it does not practice medicine, nursing, or any licensed health profession.

2 No Clinical Judgment

The questions, structure, and outputs of MindLedger's intake form are not a substitute for professional clinical evaluation. Specifically:

• Safety screening questions are designed to prompt conversation — they are not validated risk assessment instruments
• Symptom checklists are designed to organize patient-reported experience — they do not constitute a diagnostic assessment
• Mood and functional ratings reflect patient self-report only — they do not replace clinician observation
• PDF summaries are organizational tools — they are not clinical notes, assessments, or diagnoses

All clinical interpretation and decision-making remains the sole responsibility of the licensed provider.

3 For Providers

By using MindLedger, providers acknowledge that:

• They are using the platform as a documentation aid, not a clinical decision support system
• They will apply independent professional judgment to all patient information collected through the platform
• They remain fully responsible for the clinical care of their patients
• MindLedger does not establish a provider-patient relationship of any kind

4 For Patients

If you are completing a MindLedger intake form as a patient, please be aware:

• Your responses are shared directly with your provider — MindLedger is a conduit, not a care provider
• MindLedger cannot provide clinical guidance, crisis intervention, or emergency response
• If you are experiencing a mental health emergency, please call 988 (Suicide & Crisis Lifeline) or 911, or go to your nearest emergency room

5 No Warranty

MindLedger is provided "as is." We make no warranties, express or implied, regarding the accuracy, completeness, or fitness for any particular clinical purpose of the platform or its outputs. The platform may not capture all clinically relevant information, and providers should conduct thorough independent evaluations of all patients.
